The thyroid gland, as it is known, descents from the foramen cecum of the tongue to it's normal position. The ectopic thyroid gland on the midline is one of the congenital abnormalities during the embryologic development and migration of the thyroid such as descending failure. The ectopic thyroid gland could be surgically removed with the mistaken diagnosis of the thyroglossal duct cyst or myxoma. An awareness of the possibility of this congenital anomaly may prevent such as error. Recently, the authors experienced a case of infrahyoid ectopic thyroid with a cosmetic problem. So, a external lateral translocation operation was done and the result was relatively successful. Therefore we present this case with the illustration and review of the related literature.
