A clinical study on the results of middle ear surgery.

ABSTRACT

The incidence of chronic otitis media is reduced compare to that of the past. However, it still remains to be one of the most common diseases in the field of otolaryngology. Despite the development of antibiotics, the ultimate treatment for the chronic otitis media is believed to be the middle ear surgery. Although complete eradication of disease and some degree of hearing improvement was achieved, there still remains the problems with whether the patient was satisfied or not, which mainly depend upon patient's trust towards surgeon and the kind of surgery. However, surveys of these problems were not sufficient enough. The authors surveyed on postoperative satisfaction in both surgeon's and patient's point of view. The data were obtained from 150 patients who have been performed with surgery due to chronic otitis media, using patient's charts and questionnaires, which obtained 3 years after the surgery. The results were as following : 1) Among 150 patients, 72(48%) patients comprehensed well about complications of chronic otitis media in order of meningitis 38(52.8%), brain abscess 24(33.3%), labyrinthitis 8(11.1%), and facial nerve paralysis 2(2.9%). 2) Answers to the questions asking about the complications whether they have experienced were 72 patients(48%) with hearing impairment, 39 patients(26%) with recurrence, and 26 patients(17.3%) with facial nerve paralysis. 3) Answers about the symptoms that has mostly improved by surgery were 95 patients(63.3%) with reduced otorrhea, 37 patients(24.7%) with hearing gain, and 10 patients(6.7%) with reduce in pain in order. 4) According to the patient's opinion, 19.3% of patients replied with excellent, 42.7% with moderate, 34% with no change, and 4% with poor hearing improvement. 5) According to the surgern's postoperative evaluation, 52.7% replied with excellent, 44.7% with moderate, and 2.6% with poor. 6) Patients replied 31.3% with excellent, 52% with moderate, and 16.7% with poor. 7) Most common cause of poor response to the questions was poor hearing improvement. 8) Comprehensiveness in preoperative period was correlate with postoperative satisfaction rate, it was proven to be significant(p<0.01).
