Clinical diagnosis of cervical tuberculous lymphadenitis.

ABSTRACT

A clinical analysis of 181 cases of cervical tuberculous lymphadenitis which were confirmed histologically by excisional biopsy during the past 6 years from Jan. 1985 to Dec. 1990 at Wonju Christian Hospital was performed. The results were as follows : 1) The peak age incidence was in the age group of 20-29 years (34.3%), and the sex ratio of male to female was 1 : 2.6. 2) The physical findings were painless lymphadenopathy in 125 cases (68.8%), hard swelling node node in 174 cases (96.0%) and mobile swelling node in 116 cases (64.0%). 3) Multiple lymphadenopathy were seen in 80 cases (44.2%) and solitary was in 101 cases (55.8%). Of 80 cases in which there were multiple lymphadenopathy, 20 cases (11%) were bilateral. 4) The frequent location of lymphadenitis was anterior neck triangle in 98 cases (54.1%), posterior neck triangle in 75 cases (41.7%), and both triangle in 8 cases (4.2%) in order. 5) The size of 1-3 cm was 131 cases (72.3%) and over 3 cm, 50 cases (27.8%). 6) The time interval from the onset of symptom to first visit was less than 5 months in 116 cases (64.1%), 6 months to 12 months in 29 cases (16.0%), over than 12 months, 36 cases (19.9%). 7) The incidence of associated pulmonary tuberculosis was 35 cases (19.3%). 8) On contrast enhanced CT scan, multi-chambered or conglomerated nodal mass with central lucency was 16 cases (49%) and cold abscess like nodal mass, 6 cases (17%), very degree of homogenous enhancement in smaller nodes was 11 cases (34%). 9) Correlation of lymph node findings between CT scan and physical findings, single lymphadenopathy were 23 cases in physical findings whereas only 5 cases in CT scan.
