ABSTRACT |
Endotracheal intubation is common techngue for general anesthesia or patency of air way. However, inspite of its therapeutic value, several post-traumatic complications such as laryngeal edema, ulceration and granuloma are developed following intubation. From 1978 to 1984, the authors had experienced 22 cases intubation granuloma in our department and following results were obtained. 1) Age distribution showed mostly in the 20 to 49 yrs old group(86%). The sex ratio of male to female was 3 : 19, predominantly in female. 2) Main symptoms were hoarseness in 21 patients, throat clearing in 16 cases and dyspnea in 10 cases. 3) The side of lesion was bilateral in 10 cases(45%) and unilateral in 15 cases(55%). The location of mass was vocal process of arytenoid cartilage in 19 cases(86%), post 1/3 of vocal cord in 3 cases. The pedunculated mass were noticed in 17 cases and the remainders were sessile. 4) 21 cases were happened after surgery and 1 case was followed prolonged intubation due to drug intoxication. Among 21 cases, 13 cases were taken OHS and 6 cases were taken OBGY surgery. 5) Mean intubation time was 41.6 hrs ranging from 14 hrs to 121 hrs. 6) 4 cases were recovered spontaneously and others were operated with the suspension laryngoscope under the operating microscope. But granuloma was recurred in only one case.
