A Clinical Study of Intubation Granuloma

ABSTRACT

Intubation granuloma is a rare complication of endotracheal anesthesia. It is a condition, however, which is becoming more frequent owing to the increasing role of intubation in modern surgery. For 3 years, 1984 to 1986, the authors experienced 11 cases of intubation granuloma and following results were obtained. 1) All cases were female. Age distribution was ranged from 8 to 50 year of age and mostly in the 3rd-5th decade. 2) All cases had undergone endotracheal anethesia. Kinds and numbers of operations were LeFort osteotomy 3, total abdominal hysterectomy 2, thyroidectomy 2, and skin graft, Cesarean section delivery, excision of neck mass, open heart surgery were 1 case respectibly. 3) Main symptoms were hoarseness in 9 cases and dyspnea in 2 cases. 4) The site of lesions were bilateral in 3 cases and unilateral in 8 cases(left in 4 cases, right in 4 cases). 5) Duration from intubation to endoscopic removal was ranged from 2.5 months to 6 months and average was 6 months. 6) Mean time of intubation was 16.0 hours, ranged from 0.9 hours to 74.6 hours. 7) Composition of used intubation tube were red rubber in 8, portex in 3 cases. 8) All cases were operated with the suspension laryngoscope, 2 cases under neuroleptic anesthesia, 9 cases under endotracheal anesthesia. 9) One case was recurred.
