Caldwell-Luc Operation : A Clinical Study Comparing the Effects of Local versus General Anesthesia

ABSTRACT

Chronic maxillary sinusitis that is a representative of chronic nasal diseases is most frequently occurred in age 15 to 29 years. Therefore, the most patients who need Caldwell-Luc operation are high school students who are busy preparing for the admission exam. The authors thought that the time will be saved in case that bilateral Caldwell-Luc operation is carried under general anesthesia. We studied the differences between the general and the local anesthesia group in age & sex distribution, time of anesthesia & operation, amount of blood loss, hospital day, postoperative symptoms and total hospital cost. This study included 117 cases that bilateral Caldwell-Luc operation was carried in among 224 patients who were diagnosed as bilateral chronic maxillary sinusitis in ENT dept. of Ewha Womans University Hospital during the period of January 1982 through December 1986. The obtained results were as follows : 1) In age distribution, 20-29 age group was most common in the whole cases, 15-19 age group was most common in the general and 20-29 age group was most common in the local anesthesia group. 2) The sex ratio of male to female was 1.1 : 1 which revealed no specific differences. 3) In the whole cases, the local anesthesia group versus the general anesthesia group ratio was about 4.1 : 1. 4) Under the general anesthesia, mean time of anesthesia & operation were each 75.7 min and 56.3 min and mean amount of blood loss was 295.2 ml. 5) Average hospital day was 9.6 days in the general anesthesia group and 15 days in the local anesthesia group. 6) Among the postoperative symptoms, facial edema and orbital ecchymosis were more common in the local anesthesia group and toothache, fever and hemorrhage were more common in the general anesthesia group. 7) Total hospital cost revealed no specific differences in relation to anesthesia method.
