A Clinical Study of Papilloma of the Upper Airway in Children

ABSTRACT

The authors attemped a clinical analysis of 50 pediatric patients under the age of 15 who had visited the Department of Otolaryngology, Seoul National University from 1972 to 1986 and had been operated on under clinical impression of papilloma. The results are as follows ; 1) There were 18 males(45%) and 22 females(55%), showing marginal predominance of female patients. Althought patients were seen in all age groups, there were 30 patients(75%) in the 1 to 5 age group, occupying the majority. 2) There were 34 patients(85%) with laryngeal involvement, among which 12 patients(35.2%) had lesions confined to the glottis or to the supraglottis and marked the majority. 3) The commonest presenting symptoms in patients with laryngeal papilloma at the time of admission were voice change, 31 cases(77.5%). 4) Tracheostomy was performed in 12 cases(30%). 5) Papilloma of the upper respiratory tract has a propensity of frequent reccurence after seemingly definitive surgical treatment. When viewed in this regard, 21 cases(52.5%) required only one surgical procedure, and 19 cases(47.5%) needed more procedures. 6) With respect of histopathological findings, 6 cases(15.0%) exhibited some from of atypism, and in the remaining 34 cases(75%) no such finding was observed. 7) All cases had multiple lesions, in which multiple local types were seen in 13 cases(32.5%) and multiple diffuse types were seen in 27 cases(67.5%). 8) In 30% of the patients, the postoperative course was complicated with the development of hoarseness and/or web formation.
