Factors Contributing to Increased Incidence of Complications Following Surgery for Laryngeal and Hypopharyngeal Malignancy

ABSTRACT

Among complications followed by operation for laryngeal and hypopharyngeal malignant tumor, pharyngocutaneous fistula and dysphagia are frequently troblesome in both patients and surgeons. But these are controllable by understanding of the factors which influence the development of them. The authors have analyzed retrospectively which factors were concerned in pre-intra-and postoperatively in 75 cases of carcinoma of the larynx and hypopharynx who underwent total laryngectomy with or without partial pharyngectomy and/or radical neck dissection at Pusan National University Hospital from Jan. 1978 to Nov. 1987. The results obtained were as follows : 1) In case of pharyngocutaneous fistula postoperatively, the only statistically significant factors were those of postoperative low hemoglobin level(p=0.009), preoperative low hemoglovin level(p=0.031) and concomitant radical neck dissection(p=0.042). 2) In post-laryngectomy dysphagia, the only significant factor was repair of pharyngeal segment with nonabsorbable suture materials(p=0.046).
