A Study on Grafts and Implants Materials in Augmentation Rhinoplasty

ABSTRACT

Augmenting the saddle nose is one of the most popular aesthetic procedures in orient. Various materials have been used for augmenting the nasal dorsum. However, many problems may arise from augmentation rhinoplasty using foreign materials, hence necessitating sometimes secondary correction. Augmentation rhinoplasty has been performed in 31 patients with a saddle nose deformity, using silastic block in 15 cases, combined silastic and autologus cartilage in 5 cases, tragal and septal cartilage in 5 cases, and costal cartilage in 6 cases. The following results were obtained. 1) The silastic block can be used with ease, however complication rate were higher than other implant materials. Three cases of dislocation, 1 case of abscess formation, and 1 case of extrusion took place when the silastic blocks were used. 2) The complication was occured only 1 case when the costal cartilage was used for massive augmentation, however, this technique required another incision on the chest and the implanted cartilage tended to bend from the side of stresses. 3) Septal and auricular cartilage did not offer sufficient amount of tissue, when the septal and the auricular cartilages for implant were used, however, there was the least chance of postoperative displacement. They were easy to handle and gave satisfactory results in patients who were reluctant to foreign implant materials.
