Clinical Analysis on Surgical Treatment of Ranula

ABSTRACT

Ranulas are mucous retention cysts and they develop from the extravasation or retention of mucus after trauma to the sublingual gland or one of the minor salivary glands. The different methods of treatment for oral plunging ranulas have been used and these include excision of the ranula only, marsupialization with or without cauterization, cryosurgery, removal of the sublingual gland combined with excision of the ranula via an intraoral or cervical approach. Despite these treatments, many patients have experienced recurrence and sometimes, larger lesions have occurred, eg. conversion of simple ranula into plunging ranula. This study was designed to evaluate clinical features and treatment rasults according to different methods. The results were as follows : 1) Sex ratio between male and female was 8 : 14. 2) 13 of the lesions were on the right side with 9 being on the left. 3) The plunging ranula was in one of 22 cases. 4) Methods of treatment were excision only(4 cases), marsupialization (5 cases), removal of the sublingual gland combined with excision of the ranula (13 cases), and the recurrence rate following each surgical methods was 20%, 50% and 0%, respectively. In conclusion, we recommend the remocal of the sublingual gland combined with excision of the ranula as most reliable surgical method.
