A Clinical Analysis of Nasopharyngeal Carcinoma

ABSTRACT

Nasopharyngeal carcinoma shows rather poor prognosis because of the late discovery of the disease, diffculties in surgical treatment and high recurrence rate. The rapid growth and high metastasizing begavior of this cancer accounts for the high rate of treatment failure despite its marked radiosensitivity. We report the results of an analysis about 74 patient diagnosed as nasopharyngeal carcinoma and 34 patients of this who were treated with radiation therapy in National Medical Center from January, 1959 to December, 1995. The results were as follows : 1) Among 74 patients, 53 cases were male and 21 cases were female(M : F=2.52 : 1), mean age at diagnosis was 49.4 years old. 2) Presenting symptoms and signs were cervical mass in 55 cases(74.3%), ear symptom in 48 cases(64.9%), nasal symptom in 50 cases(54.1%), headache in 10 cases(13.5%), neurologic symptom in 12 cases(16.2%). 3) The mean duration from onset of initial symptom to diagnosis was 9.6 months. 4) Histopathological findings were squamous cell carcinoma in 27 cases (36.5%), non-keratinizing carcinoma in 4 cases(5.4%) and undifferentiated carcinoma in 43 cases(58.1%). 5) According to TNM system of AJCC, there were 61 cases(82.4%) of stage IV, 8 cases(10.8%) of stage III, 5 cases(6.8%) of stage II. 6) Only 34 cases were treated by radiation or radiation plus combination chemotherapy. 7) Overall 5 year survival rate was 55.6%.
