Thirty four patients of BPPV treated at Kyung Hee Medical Center from Feb. 1987 to Apr. 1989 were analyzed on detailed clinical and electronystagmographic finding. The following results were obtained ; 1) Women were afflicted more than men with the ratio of 2 : 1(23 women : 11 men) By age distribution, most of the patients(18 cases, 53%) were from 6th to 7th decade. 2) BPPV occurred on awaking in the morning in 9 cases(26.5%), changing head position in 6 cases(17.6%), during sleep in 6 cases(17.6%), specific head position in 4 cases(11.8%), and erect & lying down in 2 cases(6%) respectively. 3) Among the 34 patients, 21 cases(61.7%) felt rotating sense, 3 cases experienced floating sense. In the continuity with the provocative head position, there were 6 cases(18.6%) which showed decreased vertigo & nystagmus. 4) Accompanied symptoms during the vertigo were nausea in 10 cases(29.4%), vomiting in 8 cases(23.5%), headache in 4 cases(11.8%), and tinnitus in 3 cases(9%) in order. 5) Etiologic conditions were unknown in 32 cases(94%), but middle ear mastoid infection were observed in 2 cases(6%). 6) Preceding or simultaneous diseases were seen in 10 cases as follows : hypertension(4 cases), pulmonary tuberculosis(2 cases), otitis media(2 cases), angina pectoris(1 case), and hyperthyroidism(1 case). 7) Hearing loss was present in 7 cases(20.6%), being bilateral SNHL in 6 cases and moderate conductive HL in 1 case. 8) Direction-fixed positional nystagmus was demonstrated in 22 cases(64.7%) during the period of vertigo. Simultaneous caloric test showed bilateral normal response in 32 cases and unilateral depressed response in 2 cases. 9) In all cases, the findings of tympanic membrane and ABR were normal.
