A Clinical Study of Deep Neck Space Infections

ABSTRACT

Deep neck space infections, which affect soft tissue and fascial compartment of the head and neck, were more frequently encountered in the preantibiotic era. These infections may still result in significant morbidity or motality despite the availability of antibiotics. Authors have studied 13 cases of these infections from Jun. 1986 to May 1988. The following results were obtained. 1) Age of the patients varied from 1 to 80-year-old and sex ratio of male to female was 3.3 : 1. 2) Sites of infections were submandibular space 5 cases(38.5%), lateral pharyngeal space 5 cases(38.5%), and retropharyngeal space 3 cases(23.0%). 3) Origins of infections were considered as dental disease 4 cases(30.7%), acute tonsillitis 3 cases(23.1%), trauma to oral cavity or nasopharynx 2 cases(15.4%), acute epiglottitis 1 case(7.7%), chronic otitis media with cholesteatoma 1 case(7.7%), and unknown origin 2 cases(15.4%). 4) C-T scan and lateral soft tissue roengenogram of the neck were invaluable to initial evaluation of lateral pharyngeal space and retropharyngeal abscess, respectively. 5) The isolated pathogenic organisms commonly involved were hemolytic streptococcus 5 cases(62.5%), and staphylococcus aureus 2 cases(25.1%), which were sensitive to penicillin, erythromycin and cephalosporin. 6) Without any remarkable complications, all cases were treated with a) Surgical drainage 8 cases(61.5%) including tracheotomy 3 cases(23.1%) and antibiotic therapy and b) Only antibiotic therapy 5 cases(38.5%).
